Information for new students.

 

HEALTH DATA FORM

ALL new undergraduate students and graduate students, including business school students, are required to submit a properly completed Health Data Form (HDF) to Student Health.

  • If this is not done then the student will not be able to register for classes.
  • The form must be completed in advance, prior to orientation week. The HDF cannot be completed by Student Health at Rice.
  • Student Health cannot provide immunizations for entering students.
  • The HDF is sent to all undergraduates in the acceptance package mailed by the Admissions Office. For graduate students the HDF is mailed to you by your department.
  • Do not wait until the last minute to complete the HDF and double check that all of the required elements (immunizations and physical exam sections) are PROPERLY completed. Every year a small percentage of students either wait until after they arrive at Rice for O-week or submit a HDF that is missing information on required immunizations, or fail to notice that a booster immunization is needed. This causes delays in registration. This may cause you to be unable to register for a paricular class, or session, if that class fills up.
    • If there are problems or delays please do not become upset with the staff at Student Health. It is your responsibility to make sure the HDF is completed properly and submitted in a timely fashion.
  • The HDF may also be downloaded in PDF format. NOTE: there is no "verification card" for the PDF version of the HDF.

  • If the student will be less than 18 years of age as of the first day of Orientation Week then please download and fill out an underage consent form.
  • TIP: It is generally a good idea to make a copy of the HDF, and bring it with you to Rice, before sending it to us in the mail.
