Use this form only to report the scores of games with Play-By Dates.

Non-College Sports

Match Information

To submit scores, make sure only the designated player is reporting the scores. Use the first box in each column for the first set, the second box for the second set, etc. If you are reporting a score that only has a final score, use the first box only (e.g., for billiards, if a player won 3 games to 2).

College Sports
